POSITION SUMMARY

A licensed practical nurse (LPN) provides care and treatment in accordance with physician orders. Works in a team environment and within scope of practice as defined by State. The LPN works under the direction of physicians and registered nurses (RNs). The primary responsibilities of your role in your assigned location is to provide direct nursing care to residents and to supervise the day-to-day nursing activities following the current federal, state and local standards, guidelines and regulations that govern long term care and skilled nursing facilities. You are entrusted to ensure that the highest degree of quality care will always be provided to our residents.

KNOWLEDGE, SKILLS, ABILITIESQUALIFICATIONS

  • Knowledge of computer systems including Point Click Care
  • Knowledge of nursing and medical practices, procedures and standards of care
  • Graduate of an accredited school of Nursing.
  • Valid State licensure as an LPN
  • Current CPR certification.
    Experience: One-year experience in a long-term care environment (preferred)
    License: Current, unencumbered, active license to practice as an LPN in Colorado, CPR Certification

Responsibilities
  • Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.
  • Administers medications and treatments.
  • Participates in the care planning process.
  • Supervises LPNs and certified nursing assistants.
  • Communicates with physicians regarding changes in resident’s conditions, diagnostic test results, etc.
  • Documents assessments and care in compliance with standards of care and company policy.
  • Educates patients and their families on health-related issues.
  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.
