The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team. Requirements Graduate of an accredited Licensed Practical Nurse educational program Current Licensed Practical Nurse / LPN license within the state of practice Current CPR certification Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills Strong initiative, follow through, process orientation, attention-to-detail Reliable transportation, valid driver’s license and current automobile insurance

Responsibilities

As a home health LPN, the primary responsibility is conducting one-on-one patient care and family teaching within the home setting. This includes administering skilled nursing care as prescribed and ensuring timely documentation and collaboration with the interdisciplinary team.
