Hospice of the Piedmont is seeking a dedicated and skilled Licensed Practical Nurse to provide personal care services to Hospice House patients. Responsibilities: Direct Patient Care: Provides nursing care to Hospice residents and oversees nursing assistants. Team Collaboration: Works with the team to identify patient care needs. Communication: Reports changes in residents' conditions to relevant medical staff. Documentation: Maintains accurate patient records. Support Tasks: Assists with food preparation, housekeeping, and completes various administrative tasks. Policy Adherence: Follows personnel policies and maintains safety and infection control standards. Medication Management: Manages medication orders, performs narcotic counts, and ensures proper medication handling. Qualifications, Skills, and Abilities: Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia. Two years’ relevant nursing experience required. Ability to manage all aspects of terminal care, health team service coordination, and documentation. Emotionally mature and caring individual willing to give support and assistance to terminally ill and their families. Basic to intermediate proficiency in word processing software. Driver’s license, personal automobile, and current automobile liability insurance. Typical Physical Demands/Working Conditions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of the job, the employee is regularly required to use hands to handle, feel objects, tools or controls, reach with hands and arms; stoop, kneel, crouch, or crawl; and talk to hear. The employee frequently is required to stand, sit and climb or balance. The employee must occasionally lift and/or move up to 50 pounds and push up to 100 pounds in a wheelchair. Noise level is low. Physical and emotional health is necessary for the accomplishment of position duties. Due to the nature of services provided, it may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care.

Responsibilities

Provide direct nursing care to hospice residents and oversee nursing assistants while maintaining accurate patient records. Coordinate with the medical team to identify patient needs and manage medication orders and narcotic counts.
