LICENSED PRACTICAL NURSE (LPN) / REGISTERED NURSE (RN)

Under the general supervision of the MANAGER and/or REGISTERED NURSE (RN) and /or designate. As a home Support Supervisor, the LPN will join the Nursing team to fulfill the mandate of Compassion HomeCare to provide safe and effective support services.
The supervisor works with a wide range of clients within the community setting needing acute care, recovery, chronic disease management, disability support and palliative care.
Quality services are ensured by working with clients and families in the planning, implementation and evaluation of their homecare goals.

QUALIFICATIONS, REQUIREMENTS AND SKILLS:

  • Graduate of an approved program for LPN or RN
  • Current active registration with CLPNNL.
  • Current First Aid certificate.
  • Demonstrates leadership and supervisory skills.
  • Ability to organize and manage a diverse workload and complete supervision by home and telephone visits.
  • Ability to work as a member of a multidisciplinary team.
  • Ability to work independently with proven critical thinking and decision making.
  • Familiarity with home safety and infection control standards and procedures.
  • Understanding of confidentiality obligations and evidence-based nursing practices.
  • Good knowledge of computer use/ data entry.
  • Strong physical condition and stamina.
  • Excellent communication and interpersonal skills.
  • Compassionate and able to handle busy and stressful work environments.
  • Willing to travel within the community.

Responsibilities
  • Collaborate with Registered Nurse for all new referrals to determine the most appropriate care supervisor assignment.
  • Provide supervision of assigned clients, within scope of practice and required service delivery goals, for initial visits and quarterly and as required.
  • Consult with a Registered Nurse when changes in client’s status are assessed or needs identified outside the scope of practice.
  • Prepare monthly reports for Manager and / or designate.
  • Document all home and telephone visits completed with client / family and interdisciplinary team in an IT note.
  • Complete required documentation for assigned clients at initial home visit.
  • Update required documentation for assigned clients annually, or when changes identified.
  • Initiate client care plans and chart for home support workers at initial home visit. Update client care plans annually and as needed.
  • Perform vital sign measurements as necessary and record in client file.
  • Facilitate teaching related to care needs and safety to home support workers, clients and /or families as required.
  • Maintain an updated list of medication on client’s file.
  • Liaison with multidisciplinary team members as needed to update client status.
  • Refer RHA clients and WPNL clients to discuss concerns or questions relevant to approved services and support, with their assigned case manager or team members.
  • Collaborate and communicate with case managers, physicians, pharmacists, and other teams’ members.
  • Refer client to RHA OT for equipment needs.
  • Review assessments, referral, and service provider plans from RHA.
  • Network with community partners.
  • Facilitate workshops and education sessions for home support workers as required.
