Description

Role and Responsibilities Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse. Essential Functions Under the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse: * Provide health education according to the teaching plan established by the RN. * Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN. * Observe signs and symptoms, and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager. * Document any services rendered to the patient, changes noted in the patient’s condition, and/or family and home situations. * Document communication with RN Case Manager and physician. * Other duties as assigned. Education Requirements * Associate’s degree required. * Diploma as a Licensed Practical Nurse from an accredited program. Qualifications and Requirements * Current, valid LPN license issued from NYS Department of Education. * Experience in home health care preferred. * Community health experience preferred. Work Environment The LPN is primarily in a non-office setting and may be exposed to outdoor conditions. The working conditions are classified as medium work: * Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing. Physical Requirements The following is a description of the physical requirements on a daily basis for the LPN.
