Key Responsibilities Provide skilled, patient-centered nursing care under the direction of the supervising RN and in alignment with individualized care plans. Administer medications and treatments safely and accurately, following physician orders and agency protocols. Monitor, recognize, and report changes in patient condition with clarity and timeliness. Educate patients and caregivers on treatments, self-care, safety, and disease management. Document clinical care thoroughly and efficiently in the EMR. Collaborate with interdisciplinary partners to ensure coordinated, high-quality care. Navigate varied home environments while upholding safety, professionalism, and infection-control standards. What We’re Looking For Current, unrestricted LVN/LPN license in the state of practice. One year of clinical experience preferred (home health experience ideal, but growth-ready candidates welcome). Strong communication skills, professionalism in the home, and confidence educating patients and caregivers. Ability to balance documentation needs with real-time patient care. Comfortable working independently while remaining a strong partner to the RN and interdisciplinary team. Current CPR/BLS certification, valid driver’s license, and reliable transportation. Requirements Qualifications Graduate of an accredited school of practical or vocational nursing. Current, unrestricted LPN/LVN license in the state of practice. Minimum of one year of clinical nursing experience, preferably in home health, hospice, or acute care. Current CPR (BLS) certification required. Demonstrated knowledge of nursing principles, clinical techniques, and infection prevention practices. Proficiency in electronic medical records (EMR) documentation and basic computer applications. Strong communication, critical thinking, and time-management skills. Ability to work independently while collaborating effectively as part of the interdisciplinary team. Commitment to confidentiality, professionalism, and compliance with CareM policies, HIPAA, and regulatory standards. Valid driver’s license, automobile insurance, and reliable transportation. Physical Requirements Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
