Description Job Summary: The LPN Nurse Supervisor is responsible for overseeing day-to-day nursing operations in the Health Care Center and providing clinical support to assisted living and memory care, as needed. Working under the direction of the Director of Nursing, this role helps ensure the delivery of safe, person-centered care by coordinating daily staffing and workflow, supervising nursing team members, supporting regulatory compliance, and responding to resident care needs in a timely and professional manner. The LPN Nurse Supervisor also serves as a clinical resource for frontline staff, promotes effective communication among residents, families, physicians, and interdisciplinary team members, and assists with admissions, rounds, documentation, audits, and follow-up on changes in condition. This position requires sound judgment, strong organizational skills, and the ability to balance direct resident care responsibilities with team leadership and operational oversight. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: Employee will apply skills to ensure Person Centered Care as exhibited through our Pillars of Excellence: PEOPLE STEWARDSHIP SERVICE QUALITY GROWTH Clinical Oversight and Resident Care • Provide direct nursing care, administer medications and treatments as ordered, and ensure services are delivered within the LPN scope of practice. • Monitor resident condition, identify and document significant changes, and communicate updates to the appropriate supervisor, physician, resident representative, and care team. • Participate in rounds, admissions, assessments, and care coordination activities to support continuity and quality of care. • Promote resident rights, dignity, privacy, safety, and a clean, homelike environment in all areas of practice. Staff Supervision and Team Support • Supervise, support, and provide day-to-day guidance to nursing staff, including CNAs and licensed nurses, in accordance with established policies, procedures, and quality standards. • Coordinate staffing schedules, address urgent staffing concerns, and assist with assignment adjustments to maintain safe resident care coverage. • Serve as a clinical resource and role model for team members by reinforcing expectations for quality care, professionalism, customer service, and accountability. • Support orientation, training, and ongoing development of nursing staff in collaboration with the Director of Nursing and leadership team. Documentation, Communication, and Compliance Maintain accurate, timely, and thorough documentation in the electronic health record and ensure follow-up on resident care needs and physician orders. Respond to resident, family, physician, and team member concerns in a professional manner and escalate issues to the Director of Nursing when appropriate. Assist with audits, incident follow-up, quality improvement efforts, and regulatory readiness activities to support compliance with organizational and licensing standards. Participate in safety initiatives, department meetings, and other assigned duties that contribute to efficient operations and high-quality resident care. Requirements JOB REQUIREMENTS: • Graduate of an accredited practical nursing program required; additional supervisory or leadership training preferred • Current Illinois Licensed Practical Nurse (LPN) License in Illinois • Basic Life Support (CPR) • Minimum two years of nursing experience required, including experience in long-term care, skilled nursing, or a supervisory/charge nurse role preferred KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: • Understanding of Point Click Care or similar systems. • Experience in long-term care or assisted living recommended. • Knowledge of monitoring medication. • Excellent communication and interpersonal skills. • Ability to work independently. • Proficient with Microsoft Office Suite or related software. • Solid organizational skills including attention to detail and multi-tasking skills. • Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees. ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also is often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment. OSHA Exposure Category: Category I – Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks. INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure.
