Description This position provides quality nursing oversight, linkage of residents to primary care providers and/or mental health services, and monitors medication compliance of residents on a daily basis. Responsibilities Implements medication policies and procedures and ensures compliance with OASAS 820 Regulations. Initiates and coordinates initial referrals with primary medical doctors and mental health providers and assists when necessary. Assists with follow-up phone calls and advocacy on the resident’s behalf when necessary. Maintains knowledge of the individualized care plan for residents and provides support to the residents according to their care plan. Collaborates with other clinical and medical staff members in advocating for residents during Medicaid managed care and insurance company reviews. Accurately completes medical documentation for each resident. Provides basic First Aid and CPR when necessary and maintains current certifications. Requirements One year’s experience practicing as a Licensed Practical Nurse. Requires an unrestricted New York State License or Certification: Licensed Practical Nurse, CPR and First Aid Certified.
