Requirements Graduate of an accredited Licensed Practical Nurse educational program Current Licensed Practical Nurse / LPN license within the state of practice Minimum of one (1) year of Licensed Practical Nurse experience in private duty, home health, hospice, hospital or skilled nursing facility; previous private duty / home healthcare experience preferred Current CPR certification Reliable transportation, valid driver’s license and current automobile insurance
