This role promotes Person-Centered Care, supervises daily resident care, and supports the professional growth of staff. What You’ll Do * Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender * Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed * Use evidence-based practices to ensure safe, high-quality care delivery * Assist in managing personnel through performance evaluation, Care Chats, coaching, counseling, education, and corrective action as needed * Complete all required documentation accurately and in a timely manner * Participate in resident transitions of care, care conferences, and care plan reviews * Collaborate with Cedarwood staff to ensure optimal resident outcomes * Comply with Infection Control practices * Ensure hand-held communication equipment is maintained for effective call bell management and team communication * Attend and complete all required in-services, training, and team meetings * Abide by the organization’s code of conduct * Perform other duties as assigned What We’re Looking For * Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania * High school diploma or equivalent * Prior experience in geriatric nursing preferred * BLS Healthcare Provider certification * Basic computer skills * Strong communication skills, both verbal and written * Ability to role model professional appearance and demeanor * Demonstrates respect, integrity, compassion, and excellence * Commitment to person-centered care initiatives * Conflict resolution skills and ability to coach and support staff * Flexible schedule and willingness to work extra hours as needed * May be asked to assist in emergencies and resident evacuations Why You’ll Love Working at Frederick Living At Frederick Living, you’ll be part of a purpose-driven team committed to compassion, respect, and integrity.
