Minimum Qualifications: * Current and valid Licensed Practical Nurse (LPN) license in the state of employment. * Completion of an accredited practical nursing program. * Basic Life Support (BLS) certification. * Demonstrated knowledge of nursing principles, patient care techniques, and medical terminology. * Ability to pass background checks and comply with healthcare facility policies. Preferred Qualifications: * Experience working in a hospital, long-term care, or outpatient clinical setting. * Additional certifications such as IV therapy, medication administration, or wound care. * Familiarity with electronic health record (EHR) systems. * Strong interpersonal and communication skills tailored to diverse patient populations. * Continuing education or training in specialized nursing areas relevant to the employer’s patient demographics. Responsibilities: * Provide direct patient care including monitoring vital signs, administering prescribed medications, and assisting with wound care and other treatments as directed. * Collaborate with registered nurses, physicians, and other healthcare professionals to implement and update individualized care plans. * Document patient information accurately and timely in medical records, ensuring compliance with healthcare regulations and confidentiality standards. * Assist patients with activities of daily living such as bathing, dressing, mobility, and feeding, promoting dignity and comfort. * Respond promptly to patient needs and emergencies, escalating concerns to appropriate medical staff when necessary. * Educate patients and their families on health maintenance, medication management, and post-discharge care instructions. * Maintain a clean, safe, and organized work environment by adhering to infection control protocols and safety guidelines. Skills: The required skills enable the LPN to perform clinical tasks such as medication administration, patient monitoring, and documentation with precision and care. Effective communication skills are essential for interacting with patients, families, and healthcare teams to ensure coordinated and compassionate care delivery.
