Minimum Qualifications: * Current and valid Licensed Practical Nurse (LPN) license in the state of employment. * Completion of an accredited practical nursing program. * Basic Life Support (BLS) certification. * Demonstrated knowledge of nursing principles, patient care techniques, and medical terminology. * Ability to pass background checks and comply with healthcare facility policies. Preferred Qualifications: * Experience working in a hospital, long-term care, or outpatient clinical setting. * Additional certifications such as IV therapy, medication administration, or wound care. * Familiarity with electronic health record (EHR) systems. * Strong interpersonal and communication skills tailored to diverse patient populations. * Continuing education or training in specialized nursing areas relevant to the employer’s patient demographics. Responsibilities: * Provide direct patient care including monitoring vital signs, administering prescribed medications, and assisting with wound care and other treatments as directed. * Collaborate with registered nurses, physicians, and other healthcare professionals to implement and update individualized care plans. * Document patient information accurately and timely in medical records, ensuring compliance with healthcare regulations and confidentiality standards. * Assist patients with activities of daily living such as bathing, dressing, mobility, and feeding, promoting dignity and comfort. * Respond promptly to patient needs and emergencies, escalating concerns to appropriate medical staff when necessary. * Educate patients and their families on health maintenance, medication management, and post-discharge care instructions. * Maintain a clean, safe, and organized work environment by adhering to infection control protocols and safety guidelines. Skills: The required skills enable the LPN to perform clinical tasks such as medication administration, patient monitoring, and documentation with precision and care.
