Position Summary Responsible for assessment and counseling and coordination of community services/referrals. Acts as an advocate for patients in the BJHCHS, Inc. health system. Maintains pertinent, accurate, timely, documentation in clinical record. Essential Duties and Responsibilities * Perform accurate assessment and treatment planning through evaluation of clinical symptoms, development and personality disorders, psychosocial stressors, and global function * Provide effective counseling/case management services to patients * Coordinate care of patients with healthcare providers, community agencies, specialists * Coordinate and plans with outside agencies and home care nursing staff for post-discharge care * Maintain timely, accurate documentation on all medical records in compliance with established guidelines and policies * Demonstrate emotional resilience and stability * Demonstrate excellent communication skills with patients and staff * Capable of working with children and adults * Perform other related duties as assigned by management * Travel to various BJHCHS, Inc. medical center locations as needed and directed by the supervisor. Supervisory Duties and Responsibilities * This job may have supervisory responsibilities specifically for individuals with an LISW-CP. This need is dependent upon the needs of the department. This will be determined by the department supervisor. Qualifications * Master’s Degree in Social Work * Current SC license as LISW/LCSW/LMSW * A minimum of 3 years clinical experience providing direct services to adults, children, adolescents and families * Minimum of 2 years case management and counseling experience * Current knowledge of evidence-based models for mental health and substance abuse treatment * Ability to deliver validated evidence-based therapy services to patients * Able to develop treatment plans and document patient contact and progress * Possesses a valid South Carolina driver’s license and clean driving record

Responsibilities

The Licensed Social Worker is responsible for performing clinical assessments, developing treatment plans, and providing counseling and case management services. They also coordinate patient care with healthcare providers and community agencies while maintaining accurate clinical documentation.
