LTC COMMUNITY TEAM LEAD We are seeking a dedicated and experienced LTC Community Team Lead to oversee and coordinate our long-term care community initiatives. The ideal candidate will have strong leadership skills, a passion for improving the quality of life for residents, and the ability to manage a multidisciplinary team effectively. KEY RESPONSIBILITIES * Lead and manage the LTC community team to ensure high-quality care and services for residents. * Coordinate daily operations within the long-term care community, ensuring compliance with regulatory standards. * Develop and implement care plans in collaboration with healthcare professionals, residents, and families. * Provide training, guidance, and support to team members to promote professional development and teamwork. * Monitor and evaluate the effectiveness of care programs and community initiatives, making improvements as needed. * Serve as a liaison between residents, families, healthcare providers, and administrative staff. * Ensure accurate documentation and reporting in accordance with organizational policies and regulatory requirements. * Promote a positive and inclusive community environment that supports resident well-being and engagement. QUALIFICATIONS * Bachelor’s degree in Nursing, Healthcare Administration, Social Work, or related field preferred. * Minimum of 3 years experience in long-term care or community health settings. * Proven leadership and team management skills. * Strong knowledge of long-term care regulations and best practices. * Excellent communication, interpersonal, and organizational skills. * Ability to handle multiple priorities and work effectively in a fast-paced environment. * Compassionate and resident-focused approach to care.
