LTC OUTPATIENT THERAPIST We are seeking a compassionate and skilled LTC Outpatient Therapist to join our team. The ideal candidate will provide high-quality therapeutic services to long-term care (LTC) patients in an outpatient setting, helping them improve their physical, emotional, and mental well-being. RESPONSIBILITIES * Conduct comprehensive assessments of patients’ physical, emotional, and cognitive needs. * Develop individualized treatment plans based on patient assessments and goals. * Provide evidence-based therapeutic interventions to LTC patients in an outpatient setting. * Monitor patient progress and adjust treatment plans as necessary. * Collaborate with interdisciplinary teams including physicians, nurses, social workers, and family members to ensure coordinated care. * Document patient sessions, progress notes, and treatment plans accurately and timely in compliance with regulatory standards. * Educate patients and their families about treatment goals, techniques, and strategies for managing conditions. * Maintain patient confidentiality and adhere to ethical and professional standards. * Participate in continuing education and professional development activities. QUALIFICATIONS * Master’s degree in Physical Therapy, Occupational Therapy, Speech Therapy, Psychology, Social Work, or related field. * Valid and current licensure or certification as required by state regulations. * Experience working with long-term care patients in an outpatient or similar setting preferred. * Strong knowledge of therapeutic techniques and interventions relevant to LTC populations. * Excellent communication and interpersonal skills. * Ability to work independently and as part of a multidisciplinary team. * Proficient in electronic health records (EHR) and documentation standards. * Compassionate, patient-centered approach to care. WORK ENVIRONMENT This position primarily involves outpatient clinical work with LTC patients. The therapist will work in a healthcare facility equipped to support therapeutic services and patient care.
