Job Title: Hospice Admit RN Job Type: Full-Time Benefits: Medical, Dental, Vision, Accident, Critical Illness, Pet plans available Disability plans available Employer paid Life Insurance HSA and FSA plans Mileage reimbursement 160 hours of PTO annually plus 9 paid Holidays Job Summary: As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families. Responsibilities: Coordinates and oversees all aspects of new patient admissions Collaborates with patient care teams to ensure seamless communication Educates families on patient condition, needs, and care Performs head-to-toe physical assessments Timely and accurately completes documentation Other duties as assigned Requirements Licensure: Current State license as a Registered Nurse. Current driver’s license. Current CPR Certification. Experience: Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice preferred. Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations. Ability to work independently and make sound decisions. Strong attention to detail and organizational habits. Excellent verbal and written communication skills. Good interpersonal skills. Transportation: Reliable transportation and valid auto liability insurance.
Responsibilities
The admission nurse coordinates and oversees all aspects of new patient admissions, including performing comprehensive physical assessments. They also collaborate with care teams and educate families on patient conditions and care needs.
