Home health experience ideal, and we will provide training and support. Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team. Provide effective communication to patients, staff members, other health care professionals and referral sources. Respond to agency and patient needs in a professional and creative manner. Responsibilities

The LVN will serve as a point of contact throughout the admissions process and ensure coordination of care with other team members. They will communicate effectively with patients and provide education regarding their home healthcare experience.
