Due to growth, we are looking for an experienced Licensed Vocational Nurse to join our dynamic team serving Fresno and Madera Counties. This position is full time. Your presence and services should bring comfort to the person who faces the end of their lives and help them reach acceptance and peace of mind. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. Our goal is to help patients live their final days with dignity and comfort. Follow the plan of care and provide treatments, support and comfort according to individual patient’s needs. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care. Work collaboratively with the members of the interdisciplinary team to help meet positive patient care outcomes. Alleviate symptoms to the patient's desired level of comfort and help in administering medication. Evaluates patient’s response to treatments/medications. Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities. Notify a doctor for a patient’s condition when deemed necessary. Assist patients and their families with paperwork. Maintain accurate records. Help relatives cope with the upcoming loss and provide post-mortem support. Participate in after hours on-call, holiday and weekend rotations.
Responsibilities
The LVN provides direct patient care, including specialized duties related to end-of-life care, and collaborates with the interdisciplinary team to achieve positive patient outcomes. Responsibilities also include educating patients and families, maintaining accurate records, and participating in after-hours on-call rotations.
