Take patient vitals, do assessments, document progress and report changes to RN supervisor. Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques Collaborates with physicians, other agency staff and contract services to coordinate and implement the Plan of Care. Completes all required documentation in accordance with the standards of the agency, and regulatory requirements. Medical specialties: Home Health Standard shift: Night shift Supplemental pay types: Overtime pay License/Certification: LVN (Required) Requirements Requirements: Current CA state license to practice as a Licensed Vocational Nurse in California. A minimum of one (1) year home health experience is strongly preferred. Current BLS/CPR Certification Experience in caring for patients with G-Tubes, Trachs, and ventilators. Works in patients home in various conditions; ability to work a flexible schedule. Able to travel to prospective patient’s place of residence. Able to stoop and bend; must be able to lift and transfer patients.

Responsibilities
