Job Summary The Licensed Practical Nurse (LPN) – Wound Care provides direct and consultative patient care focused on wound management under the supervision of the Wound Care Coordinator and Registered Nurse. This role includes wound assessment, implementation of treatment plans, education, and collaboration with the healthcare team to ensure best practices and cost-effective care. Essential Functions Provide wound care to patients in accordance with physician orders and established protocols Assess wounds on admission and regularly throughout the stay to identify healing progress or complications Collaborate with RN and Wound Care Coordinator to review treatment plans Report wound status changes to the supervising RN and healthcare team Participate in patient discharge planning to ensure continuity of wound care Educate staff and patients on wound care products and proper techniques Evaluate and recommend wound care products based on clinical effectiveness and cost Maintain accurate documentation of wound assessments and care provided Act as a resource to nursing staff for complex wound cases Knowledge/Skills/Abilities/Expectations Specialized knowledge in wound assessment, treatment, and debridement Ability to lead and support team collaboration Effective communication and problem-solving skills Familiarity with infection prevention, decontamination, and dressing selection Attention to detail and ability to track wound progress through documentation Must have good and regular attendance Ability to work independently and follow through on clinical decisions Ability to lift up to 50 lbs and perform tasks requiring frequent movement Adheres to organizational values and compliance standards Education Graduate of an accredited practical/vocational nursing program Licenses/Certifications Current LPN license in state of practice or Compact State License (if applicable) Basic Life Support (BLS) certification Experience Minimum of three (3) years of clinical nursing experience, preferably in acute care or wound management
