Description

Exciting opportunity for new grads or experienced Home Support Worker (HSW), Community Support Worker (CSWs) or Personal Support Workers (PSWs) who are interested in providing exceptional and compassionate care for geriatric client(s) in Etobicoke and surrounding locations.

REQUIREMENTS:

  • Language proficiency is a bona fide occupational requirement for this position, as the client communicates primarily in this language and has limited English proficiency.
  • Recent experience in providing home care services in home/community settings or hospital/LTC settings, including clinical placement, is a must.
  • Community Support Service (CSW) or Personal Support Worker (PSW) Certificate or equivalent will be considered a strong asset.
  • Experience with dementia care will be considered a strong asset.
  • Have a clear and recent Vulnerable Sector Screening
  • Have a valid CPR/First Aid certificate, Proof of immunization (flu shot), Mask Fit Certificate and TB test
  • Must possess a valid SIN and Work Permit to work in Canada

Responsibilities

As a Professional support worker at CareHop, you will play a crucial role in assisting individuals in their daily lives while maintaining a clean and safe environment. Your responsibilities will include, but are not limited to:

  • Provide personalized assistance to individuals facing physical, emotional, or social challenges.
  • Assist senior clients with activities of daily living, such as bathing, dressing, grooming, transferring and repositioning, and foster a sense of independence.
  • Offer emotional support by being a compassionate listener.
  • Facilitate community engagement and social interaction.
  • Work closely with healthcare professionals, families, and other team members to provide exceptional care.
  • Communicate effectively to report any changes in the client’s condition or needs.
  • Maintain accurate and timely documentation of care provided.
  • Develop and implement support plans tailored to individual needs.
  • Follow established protocols for reporting incidents or concerns.
