Must have a high school diploma, preferably with at least one to two years’ experience working with HIV or OB patients. Experience in integrated health services, social determinants of health assessment, outreach, caseload management and/or tracking is a plus. Responsibilities
Community Health Navigators provide counseling, outreach, and referral services to improve the quality and utilization of HIV services for patients and their support systems. They work both independently and as part of a team across multiple healthcare sites.
