The Manager of Allied Health supports the Community Bundled Care and Integrated Care Solutions (ICS) programs by providing clinical leadership, professional practice oversight, and operational support. The Manager also plays a key role in supporting safe transitions from hospital to home, collaborating with hospital partners, internal teams, and community providers to ensure timely, coordinated, person and family centered care What You'll Do Complete intake assessments and develop individualized care plans for newly admitted patients. Provide clinical leadership and mentorship to Allied Health professionals and other interdisciplinary teams. Conduct case reviews, monitor progress, and evaluate client outcomes. Perform home visits for training, performance evaluation, and clinical support. Oversee daily operations of the Bundled Care Program, ensuring compliance with provincial legislation, regulatory standards, and organizational policies. Monitor and analyze intake processes, priority codes, refusals, and service delivery metrics. Collaborate with the Clinical Manager to track, report, and communicate program outcomes with the data management team. Implement strategies to improve workflow efficiency, service quality, and patient experience. Work closely with hospital partners, and community agencies to support safe transitions and coordinated care planning. Address patient and family concerns with professionalism and empathy. Participate in case conferences, interdisciplinary meetings, and cross‑sector planning discussions. Provide mentorship, supervision, and performance management to Allied Health staff and other clinical team members. Support ongoing staff development through coaching, training, and continuing education. Promote a culture of compassion, excellence, and reliability aligned with organizational values. What You Bring Registered Physiotherapist (PT) or Occupational Therapist (OT) in the province of Ontario, in good standing with the College of Physiotherapists of Ontario or the College of Occupational Therapists of Ontario. Master’s degree in Physiotherapy or Occupational Therapy from an accredited and recognized program. 5+ years of experience in healthcare, preferably in home and community care or hospital environments. Demonstrated knowledge of community rehabilitation, care pathways, and best practices in service delivery. Strong leadership skills with the ability to mentor, coach, and support Allied Health teams. Exceptional interpersonal skills and the ability to manage challenging situations with professionalism and consistency. Why Join Bayshore?
