QUALIFICATIONS

• Education
• Bachelor’s degree in business administration, Public Health, Social Sciences, or a related field (required).
• Graduate-level education (e.g., master’s degree) considered an asset.

THIS IS A COLLABORATIVE AND INTEGRATED ROLE BETWEEN ERIE SHORES HEALTHCARE AND CMHA WINDSOR-ESSEX.

Reporting to the Chief Executive Officer of each organization, the Manager of Community Outreach will:

• Serve as the coordinator for the Mobile Medical Support Team and oversee the day-to-day delivery of high-quality programs and services that respond to the needs of clients.
• Act as a conduit for community engagement, strengthening partnerships and outreach efforts.
• Support vulnerable populations by facilitating access to care.
• Ensure patients are connected to primary care providers to promote continuity and better health outcomes.

OTHER DUTIES

• Perform other relevant tasks as required to support the mission and objectives of Erie Shores Healthcare and CMHA Windsor-Essex.
