Description

The Scarborough Academic Family Health Team (SAFHT) is governed by a Mixed Board (Physicians + Community Members). It is a multi-site Family Health Team located within Eastern Toronto using a collaborative model to provide comprehensive primary care for rostered patients.
The Manager will be an essential member of the Scarborough Academic Family Health Team (SAFHT) and supports the team in delivering comprehensive primary care and promoting health initiatives & programs. Reporting to the Executive Director, the Manager provides oversight and clinical supervision to Interdisciplinary Health Providers (IHPs) and staff. Key responsibilities include developing, coordinating, implementing, monitoring, and evaluating SAFHT programs and services. The Manager collaborates closely with physicians, the Executive Director, IHPs, community partners, individuals and groups to ensure the effective implementation of health promotion initiatives and programs.

EDUCATION QUALIFICATIONS AND SKILLS

  • Active membership in good standing with a relevant regulatory college
  • 2-3 years of clinical experience in primary care, community health, or acute care setting, with a focus on working with vulnerable populations.
  • Strong knowledge of chronic disease management, mental health, and addiction services.
  • Excellent leadership, communication and interpersonal skills.
  • Strong commitment to patient centred care.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
  • Current CPR certification.
  • Proficiency in the use of computers and information technology, including experience with electronic medical records (EMR), Practice Solutions preferred.
  • Ability to prioritize and adapt in a fast-paced and dynamic work environment.
  • The ability to speak and/or write in more than one language is an asset.

ADDITIONAL JOB REQUIREMENTS

  • Must adhere to all SAFHT Policies and Procedures.
  • Must be legally entitled to work in Canada.
  • This position is in Scarborough, ON, the position requires working in office and on site.
  • As a condition of employment, you are required to submit an Immunization Record.
  • Current Ontario Driver’s License and motor vehicle as the position will involve driving to different SAFHT sites based on need.

Responsibilities

Program Management

  • Conduct needs assessments to identify gaps in service delivery and/or programs and make recommendations based on assessment, analysis, and best practices.
  • Implement plans and evaluation for each program area in accordance with SAFHT goals and the strategic plan.
  • Knowledge and proficiency in evidenced-based practices of primary care delivery, including disease prevention and health promotion.
  • Develop effective health communication resources for individuals or groups.
  • Utilize SAFHT website to promote resources, services and programs.

Operations Management

  • Provide clinical supervision to IHPs related to the delivery of SAFHT programs & services.
  • Assist with case consultations.
  • Manage the review and implementation of practice standards for IHPs.
  • Contribute to performance coaching with IHPs.
  • Lead discipline specific meetings
  • Ensures IHP coverage and assists with coverage when necessary.
  • Provide and/or facilitate training and education sessions to IHPs.
  • Support the privacy officer in delivery of training and prevention activities.

Quality Improvement

  • Monitor patient safety and quality issues, and ensure incident reports are completed.
  • Lead quality improvement initiatives
  • Manages patient complaints, and escalates to Executive Director as required
  • Contributes to quarterly reporting and prepares reports to communicate KPIs and quality indicators for IHPs
