San Diego’s St. Paul’s PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Full Time onsite position. The typical schedule for this role is Monday - Friday, 8:00AM - 4:30PM.
Job Summary: Manages and coordinates all aspects of St. Paul’s PACE supportive housing program including program operations oversight, acts as the liaison for housing
services in the community to ensure ongoing and successful living for the PACE participants.

QUALIFICATIONS AND REQUIREMENTS:

Education: Bachelor’s degree an accredited college or university. BSW,
psychology or equivalent relevant degree.

EXPERIENCE:

• A minimum of one year’s experience in working with the frail or elderly

population required

• Recommend at least one year experience working directly with homeless

housing services organizations, Seniors 55 plus, and knowledge of the Veterans community.

• Prefer four years of related experience and/or training or an equivalent combination of education and experience.
• Must possess strong computer skills and knowledge in the use of software application programs in word processing and spreadsheets. This is to include at a minimum such programs as Word, Excel, and PowerPoint.
• Must be willing to learn or have experience with the Coordinated Entry System (CES) Service Point and Shelter Point IT systems.
• Excellent communication skills to impart the value of all St. Paul’s services.
• Must be able to attend after hours and weekend events to secure support for the program including fundraising activities, engaging volunteers, and creating community awareness of the program.
• Bilingual in Spanish is an asset

• Must have strong interpersonal skills to effectively communicate with and

relate to Community members, Board members, participants, residents, donors, diverse Volunteers and staff.

• Must possess excellent verbal and written communication skills. Must be able to effectively create and write reports and correspondence.
• Must possess strong organizational skills to successfully plan for and meet deadlines, schedules and goals.
• Must be self motivated and not easily distracted from the task of serving this vulnerable population.

Program Management and Oversight

• Manages St. Paul’s PACE supportive housing program and staff.
• Designs and develops a comprehensive program for identifying, accessing, assessing, and coordinating PACE participants who are homeless or near homeless into permanent housing, with a strong emphasis on Veterans.
• Locates housing in San Diego County for those not assigned to PACE identified units.
• Represents St. Paul’s PACE at community meetings and provides outreach for the Housing program.
• Develops and keeps current all policies and procedures and guidelines for permanent supportive housing services for St. Paul’s PACE and all other St. Paul’s communities.
• Develops a process for accessing referrals, assessing housing needs, connecting with housing agencies, application processing, move in process, collaboration with PACE and other supportive services, and monitoring program success and quality.
• Develops, delivers tabulates and reports program participant satisfaction with their housing situation.
• Tracks outcomes of housing on the participants health and social engagement.
• Maintains computer records of all community outreach meetings & events, potential participant interactions, volunteer interactions, applicant tracking, supportive housing resident interactions, housing program meetings, and other pertinent daily activities.
• Coordinates the preparation, invitation, and follow-up for meetings and special activities.
• Coordinates recruitment, on-boarding, initial and on-going training, of volunteers for supportive housing related projects. Utilizes volunteers to support the programs.
• Coordinates with the Volunteer Coordinator and the Foundation to ensure the housing program is ready for new participants to move in.
• Represents St. Paul’s PACE at regular meetings with Housing Owner/Property Management as requested in the Memorandum of Understanding (MOU) and

additionally as needed.

• Negotiates and executes conditions of contracts with partner agencies independently.
• Maintains at least regular monthly contact with the Owner’s property manager via the phone or through a formal meeting. Agenda to include, at minimum:
• An update of anticipated vacancies and move-ins.
• Supportive housing service coordination.
• Current rent roll, PACE Unit Tenant notices serviced, behavioral issues and concerns with PACE Unit Tenants, housing quality standards issues with PACE Unit Tenants, and other service referrals.
• Manages the use of outside agencies to provide staff training.
• Represents St. Paul’s in any conflicts or concerns of any property managements.
• Attends all tenant-specific meetings, as needed.
• Collects and provides housing organizations and PACE Quality Assurance team with aggregate level data and information related to housing the PACE Unit Tenants, and any other information reasonably requested by housing organizations or PACE at an aggregate level for State and Federal reporting.
• Writes quarterly written “Evaluation Plans” as requested in any supportive

housing MOU.

• Documents Initial and ongoing CES/Service Point data for all PACE supportive housing participants
• Represents PACE on any PACE housing grants in the community. Responds to grant requests for information.
• Manages grant budgets to ensure compliance with budget controls and fiscal soundness of program.
• Coordinates with property management for pest control and ongoing issues.
• Tracks and reports identified outcomes of PACE participants in ongoing supportive housing projects.
• Assists St Paul’s foundation with fundraising efforts for furnishings.
• Coordinates purchase of, delivery, set up, and records the costs of furnishings and provide invoices/receipts for reimbursement to SPSS finance.
• Manages completion of duties and responsibilities of other assigned staff to ensure successful and timely compliance for all housing related activities.
• Provides weekly reports and meets weekly with supervisor on all activities, goals, outcomes and action steps.

Supportive Services

• Represents St. Paul’s PACE as the homeless housing liaison with all of St.

Paul’s contracted housing facilities by making regular visits to property managers as well as PACE participants.

• Incorporates the work and mission of each contracted facility and program by maintaining good relationships with St. Paul’s and PACE community staff in order to facilitate supportive relations and ensure successful supportive housing experiences for PACE participants.
• Manage all relationships with contracted community housing providers and the senior recipients of housing to ensure ongoing and successful living for the

participants.

• Engages in all efforts to ensure supportive housing tenants remain enrolled in PACE.
• Regularly checks in with tenants to identify issues of satisfaction and dissatisfaction, assists with grievance resolution related to housing issues, and makes quality program improvements.
• Participates in disenrollment prevention strategies in partnership with the PACE IDT.
• Attends and participates in PACE Level of Care (LOC), Interdisciplinary Team (IDT) and Plan of Care (POC) meetings as requested, when involving potential

and current PACE supportive housing tenants. Provides care plan recommendations to ensure successful supportive housing.

• Recommends and works with PACE Social Workers and staff to ensure Tenants receive all social & Government benefits to which they are entitled.
• Collaborates with Social Workers to actively assist with all appeal letters and/or processes related to housing application denials.
• Responds to property managers/developers concerns or requests on behalf of the PACE Tenants in a timely manner in coordination with the IDT as needed.
• Makes recommendations to PACE IDT on adequate and necessary services for specific participants in supportive housing, including mental health and substance abuse services.
• Promotes the awareness of need and opportunities for permanent housing placements in the community, to St. Paul’s staff, to current and potential participants & residents, and among constituents
• Ensures all St. Paul’s property remains within PACE Unit Tenant rooms and is removed from those who may disenroll.
• Conducts regular tours of the facilities to ensure a safe and secure environment for participants; reports results to the Property manager.
• Provides immediate notification to the Owner’s property manager prior to participant disenrollment.
• Assists with resolving any PACE Unit Tenant housing situation or lease violations.
• Performs other duties as requested by the Operations Director, Executive Director of Home and Community Services, or the Chief Executive Officer.
