How you make a difference:
The MAT Coordinator/Discharge Planner, guided by the MAT Medical Director, delivers comprehensive on and off-site services to MAT Program members, administering medical support such as injections, coordinating medications, and ensuring effective communication with psychiatric and medical providers. They play a key role in discharge planning for facility patients, conducting and implementing coordination efforts while maintaining accountability for the services provided. The coordinator fosters communication with both facility staff and community resources to ensure the seamless provision of medical services to enrolled MAT Program members.

Key Responsibilities:

  • Oversee discharge planning and coordination of Medication-Assisted Treatment (MAT) services for mental health patients, collaborating with medical, nursing, and security staff.
  • Establish and maintain relationships with community agencies offering MAT and services for mental illness, ensuring a seamless transition for patients.
  • Adhere to security regulations and confidentiality limits, identifying release planning needs 120 days before a patient’s release, and coordinating medication and medical requirements post-release.
  • Document services provided in health records, make appointments for continuing care, and engage in positive communication with Unit Team Staff, Parole Officers, and community providers.
  • Attend relevant meetings, participate in training, and represent the corporation’s interests with administrative staff to facilitate effective delivery of MAT services.

Qualifications & Requirements:

Education

  • Graduate from an accredited school of nursing

Experience

  • One year clinic experience
  • Previous experience in developing successful transition plans involving community resources for persons with mental illness

Licenses/Certifications

  • Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
  • Must be able to obtain and maintain CPR certification
