The Mobile Assessment & Treatment Service (MATS), also known as Alfred Health’s Residential In Reach service, are seeking an enthusiastic and driven Nurse, looking for an opportunity to join our dynamic and well-regarded team.

QUALIFICATIONS AND EXPERIENCE REQUIRED

  • Current registration with the Nursing and Midwifery Board of Australia via AHPRA
  • Relevant and recent clinical experience in General Medicine, Acute aged care or Emergency Nursing
  • Demonstrated ability to work positively and flexibly with internal and external services.
  • Current Victorian Driver’s Licence

Desirable

  • Post graduate qualifications in gerontology nursing and/or palliative care nursing
  • Clinical competence/credentialled in catheterisation for male/female IDC’s & SPC’s; IV Cannulation.

Responsibilities

THE ROLE

As a MATS Outreach Nurse, you will be motivated to optimise the health experiences of frail elderly patients living in residential care and community settings.

The Outreach Nurse is responsible for:

  • Working within a collaborative multidisciplinary team framework to ensure best care, whilst at times practising autonomously when providing that care
  • Conducting comprehensive and holistic systems assessments, with astute ability to detect and identify clinical deterioration
  • Delivery of safe patient centred care as determined by the multidisciplinary team and overseen by the Geriatric Consultants. Such care often includes (but is not limited to) administration of IV medications, urinary catheter care, wound assessments and management, and supporting end of life care
  • Evaluating and adjusting that plan of care in partnership with the patient and/or their decision maker, their GP and the residential care staff and other members of the health care team
  • This role works at the interface of acute/sub-acute hospital, GPs, and RACH’s to ensure integrated care for the person
  • The position requires excellent communication and documentation skills to foster effective relationships with internal and external stakeholders to the patient’s care
