The MDS/Care Plan Coordinator is responsible for coordinating the entire RAI process assuring the accuracy, timeliness and completion of the MDS, CAAs and interdisciplinary Care plan for all residents in the facility in accordance with all federal and state certification guidelines. He/she will monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members. he/she is entrusted to provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results. This position is held by an essential employee and is vital to maintaining the quality of care given to our residents and patients during the coronavirus / COVID-19 pandemic and beyond.

  • Assist in conducting and coordinating the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs and Triggers.
  • Assist in managing the overall process and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided within the facility.
  • Conducts concurrent MDS reviews to assure achievement of maximum allowable RUG categories.
  • Integrates information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.
  • Maintain resident confidentiality and privacy; treat residents with kindness, dignity and respect; know and comply with and ensure that all nursing services personnel know and comply with Resident’s Rights rules.
  • Monitor and complete all corrections on MDS
  • Provide written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees.
  • Meet with and solicit advice from department supervisors concerning the resident assessment/care plan functions of the facility.
  • Evaluate each resident’s condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
  • Other related duties as assigned.
