Position Summary: The Cypress of Charlotte is recruiting for a hospitality focused MDS Coordinator to join our team in a full-time capacity! Under the guidance of the Director of Nursing, the MDS Coordinator is an LPN or Registered Nurse who is responsible for the overall supervision, coordination, maintenance and timely completion of both the RAI and Care Management process from admission to discharge in accordance with company policy and procedures, State and Federal guidelines, and all other entities as appropriate. Primary Responsibilities: Works in collaboration with the Interdisciplinary Team to assess the needs of the resident. Coordinate and complete all state and federally required assessments, Minimum Data Set (MDS) and care plans in a timely and accurate manner. Review assessments and care plans no less than once every 90 days and more frequently as necessary. An on-going cycle of assessment intervention, care planning and evaluation will be conducted and reflected in the care plan. Complies with federal and state regulations regarding completion and coordination of the RAI process. Ensures MDS electronic submissions occur per State regulations. Effectively communicate the contents of the care plan to the resident, the resident’s family and all appropriate employees and health care providers. Performs chart audits to ensure assessments and documentation are completed, accurate and present to support MDS coding. Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident, and family members. Assists in coordination and management of the daily care management meeting, to include review of the resident care and the setting of the Assessment Reference Date. Assist in the coordination and management of the weekly Medicare meeting. Assist in the overall supervision of the nursing care in the Health Center. Monitors policies, procedures and work performance standards ensuring they meet all Federal, State and Local regulations. Assures accuracy and completeness of medical records by participating in and completing medical record audits to ensure compliance. Participates and assists with orientation of nursing personnel to the Health Center. Qualifications: Must have valid NC or compact state LPN/RN license with no substantial findings of abuse, neglect or misappropriation of resident property. CPR/BLS certification required. Minimum of 2 years of experience in LPN/RN capacity, preferably supervisory or management level. Comprehensive knowledge of nursing principles and practices. Experience with the RAI process and MDS transmittal process and understanding of regulations impacting the MDS. Maintains confidentiality of members in accordance with HIPAA and Corporate Compliance regulations. Compensation range depends upon experience. This position includes a $5,000 SIGN ON BONUS!
