Minimum Qualifications: * Bachelor's degree in Nursing, Healthcare Administration, or a related field. * Current RN or LPN license in the state of practice. * Experience with MDS assessments and knowledge of CMS regulations. Preferred Qualifications: * Certification in MDS coordination or related field. * Experience in a long-term care or skilled nursing facility. * Familiarity with electronic health record (EHR) systems. Responsibilities: * Conduct comprehensive assessments of residents to complete the MDS documentation accurately. * Collaborate with nursing, therapy, and other departments to gather necessary patient information. * Train and educate staff on MDS processes, updates, and compliance requirements. * Analyze MDS data to identify trends and recommend improvements in patient care. * Ensure timely submission of MDS assessments to meet regulatory deadlines.
