While you make a difference in the daily lives of our residents, we’re here to support you with competitive wages, comprehensive benefits, and opportunities to grow in your career.   POSITION SUMMARY: The MDS Coordinator plays a key role on our interdisciplinary team, ensuring accurate and timely resident assessments while helping optimize quality of care.   RESPONSIBILITIES: * Participate in the pre-admission process, gathering essential information for MDS optimization. * Assist in reviewing resident care and setting ARDs to ensure the most appropriate timing for PPS MDS Assessments. * Accurately complete and code MDS assessments per state and federal guidelines. * Collaborate with staff, residents, and families to gather information and ensure quality outcomes. * Maintain MDS schedules and documentation. * Perform other duties as assigned.   QUALIFICATIONS: * Current, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice. * Strong communication and interpersonal skills. * Familiarity with the RAI process and case management preferred.   WHY JOIN US?
