We are currently seeking a Full-time qualified facility LPN – MDS Nurse to join our team at Peak Resources Outer Banks! Excellent compensation and benefits with Quarterly bonus potential! Typical Work Schedule is: Monday-Friday, 8:30am-5:00pm, with occasional on-call rotations. Role Responsibilities Coordinates and facilitates the timely and accurate completion of both the RAI and Care Management process from admission to discharge in accordance with Company P&P’s, State, and Federal guidelines, and all other entities as appropriate. The MDS Nurse- LPN will work under the supervision of an RN. Requirements Required Skills, Education, Licensure, and/or Certifications Current, unencumbered, active license to practice as an LPN in NC.
