Duties of the MDS Nurse (RN): The MDS Coordinator is responsible for leading the Interdisciplinary Team Plan, organize, develop and implement the MDS assessments and care plans Update records and assessments, ensuring information is consistent with patient care plans and that scoring is accurate Accountable for accurate MDS scoring Generations at Applewood is a 24-hour rehabilitation and skilled nursing community offering quality healthcare services to senior residents throughout the Chicagoland area. Requirements of the MDS Nurse (RN): Current RN licensed from the State of Illinois American Association of Nurse Assessment Coordination (AANAC) certification preferred RAC-T Certified required Knowledge of Minimum Data Set (MDS), ICD-10 Coding Two (2) to three (3) years of applicable experience as a Nurse preferred Must be CPR Certified Experience in a long term care setting preferred Generations at Applewood is an Equal Opportunity Employer IND1
