Description DEPARTMENT: ADRC ACCOUNTABILITY: ADRC Director STATUS: Exempt, Full-time - Hybrid schedule JOB SUMMARY Under direction of ADRC Director, performs needs assessments, counseling and referrals, preliminary care planning and short-term tracking for persons referred by a Nursing Home Social Worker that have stated their interest in resources related to home and community-based services. The Options Counselor serves a large geographic area, which may include one large county, and /or many small counties. This position may involve extensive travel. DUTIES AND RESPONSIBILITIES Essential Functions Performs comprehensive needs assessments, including mental, physical, functional, and environmental to determine appropriate referrals; Provides clinical eligibility counseling and financial prescreening, for State and Federal Programs; Develops preliminary care planning, options counseling, conducts research and provides follow-up; Documents in electronic and/or paper form according to policy guidelines; Provides options counseling to select persons in Rehab facilities m Nursing homes Assists with/provides periodic public education sessions; Attends ADRC Staff Meetings; Provides information, referrals and assistance through telephone, walk-in or email intake as needed; Provides home and community visits if required Other duties as assigned. Requirements EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS Education: Registered, professional nurse currently licensed to practice in the State of Georgia or at least 4-year degree in Social Work, Sociology, Psychology or related field. Knowledge and Experience: Two years’ experience with Legacy Link preferred; minimum of two years’ experience in the areas of aging, disabilities, community health, nursing home or hospital discharge planning. Experience working with older adults and/or adults with disabilities preferred. Skills: Ability to establish and maintain effective working relationships with representatives of other social agencies, institution officials, the public, and clients and their families Ability to communicate effectively, summarize data, prepare reports and make recommendations based on findings which contribute to solving problems and achieving work objectives Ability to exercise good judgment in evaluating situations and in making decisions Knowledge of human behavior, gerontology Skills in team building and group dynamics Knowledge of community organization and service system development Problem solving skills and techniques Knowledge and skill in social and health service intervention techniques and methodology Knowledge of availability and use of community resources Proficient in current Windows operating system used by agency and Word; able to use DDS programs Well organized, detail oriented and able to work independently and/or as part of a team Ability to communicate with clients having a variety of educational levels Openness and flexibility in working with diverse groups, respecting different cultures, backgrounds, religious beliefs and demographics Licensure and Certification Valid State Driver’s License current nursing license if appropriate Will be eligible to take the CIRS-A after one year of experience. Regularly required to read documents and write neatly, legibly, and transcribe accurate information and numbers/values. Employee continually engages in activities that require talking and hearing. Frequent: Standing, sitting, walking, repetitive use of hands, simple grasping and fine hand manipulation, pushing and pulling hand. Occasionally require to lift or to reach above shoulder level. Must be able to lift up to 11 pounds, and occasionally 25 pounds. May be required to walk on uneven terrain to get to clients' homes. WORK ENVIRONMENT The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job. Temperature ranges from normal indoor climate-controlled environment in buildings or vehicles, vartous outdoor conditions and temperature extremes encountered during off-site travel, and unpredictable indoor environmental conditions encountered in client homes or other off-site localtons. Regularly exposed to outside weather conditions when traveling lo clients' homes. May be subjected to clients with communicable diseases, or may be exposed to potentially infectious matertals. During off-site travel, employee may encounter a variety of unpredictable environmental conditions in clients' homes, including unsanitary conditions. These unclean conditions may include (but are not limited to) housekeeping neglect, exposure to cigarette smoke, poor air quality, mold, mildew, fungus, spoiled food, vermin or animal feces. May be required to walk on uneven terrain to get to clients' homes. May be required to travel to community areas that are potentially dangerous, or to Isolated rural locations.
