Flexible scheduling within department hours (8:00 a.m.–4:00 p.m., seven days per week) based on patient demand. Employees may pick up additional shifts if desired.  Frequent Internal Contacts: 1. Medical facility staff Number of Employees under Supervision and Titles: 0 Job Summary: The Med Pool Community Health Nurse performs assessments, treatments, and collaborative case management of home health services. They are experienced in nursing, with emphasis on community health treatment and education. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Essential Responsibilities: * Provides services requiring substantial and specialized nursing skills, in accordance with provider orders and departmental clinical practice guidelines. * Demonstrates competency in all skills required for the agency, including, but not limited to, venipunctures, infusion therapy and device care, wound care, ostomy care, chronic disease management, and acute transitional care ensuring aseptic techniques.  * Conducts comprehensive patient assessments in accordance with agency standards of practice and scope of practice guidelines to determine patient needs and provide appropriate skilled nursing care. * Develops and implements individualized plans of care in collaboration with the interdisciplinary care team, patients, and caregivers to support patient goals and ensure coordinated care. * Monitors patient progress through ongoing reassessments, revising the plan of care as needed to address changing needs and promote continuity and quality of care. * Initiates referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so. * Identifies discharge planning needs as part of the care plan development and implementing key steps prior to discharge of the patient from departmental care. * Documents patient care in accordance with all regulations in an efficient and timely manner.  * Participates in the agency’s quality improvement program contributing to continuous departmental growth. * Other duties as assigned. Essential qualifications, knowledge, skills and abilities needed for position: Area Comments Required Preferred Education Bachelor’s Degree in Nursing (BSN) Associates Degree in Nursing (ADN) RN or Licensed Practical Nurse (LPN) ☐ ☒ ☒ ☐ Experience and Knowledge Knowledge of therapeutic methods, ability to assess complex patients and determine treatment plans.  - Required Knowledge of medical terminology, ability to understand impact of particular illnesses upon patient’s needs with implications for treatment/care requirements.  - Required Minimum two years of clinical experience - Required Minimum one year Home Health Experience  - Preferred Skills Excellent organizational, prioritization and time management skills Exceptional communication, interpersonal skills, demeanor and professional customer service Strong desire to assist and provide care for those in need Administratively competent and proficient with computers (Google suite experience preferred) and electronic health records platforms Ability to effectively communicate in written and verbal form with caregivers of other disciplines, community agencies, physicians, and patients/families and to establish professional working relations with others.  Ability to complete concise and appropriate case notes and prepare documentation according to established guidelines.  Self-directed and able to work with minimal supervision All req. Licenses or Accreditations Valid State of Colorado motor vehicle operating license, with continued safe driving history Current licensure to practice nursing in Colorado (Registered Nurse or Licensed Practical Nurse) CPR and First Aid certification All req. Computer Operations Proficient computer skills; the ability to type, keyboard, and navigate in various programs.  ☒ ☐ Language and Communication Skills Ability to communicate orally with a wide range of individuals, in various and potentially stressful situations. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
