General Summaries Department: Social Services Shift:, Status: PRN / 80 hours per pay period Shift: 8:30 - 05:00 Facility: Atrium Medical Center The medical social worker provides direct social work intervention, transition planning and counseling to patients and their families. The medical social worker assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information to the physician, nursing staff and other care team members to facilitate appropriate transition planning. The medical social worker assists patients and families in making appropriate linkages to community resources and provided consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and social work services are provided to inpatient and outpatient populations of the hospital. Qualification: Education: Master’s Degree in Social Work from a college or university accredited by the CSWE (Council on Social Work Education) required Licensure: Licensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist Board. LSW required, LISW preferred. Experience: At Least 3 years of experience in a health care setting or other social service agency. Skills/Other: Must possess strong clinical and communication skills.
