Job Details Job Location: Jawonio Inc - NEW CITY, NY 10956 Position Type: Full Time Salary Range: $18.50 - $20.00 Hourly Summary: Coordinate and assist with medical appointments. Operate agency vehicles to transport individuals between residences/agency, program, doctor appointments. Essential Duties and Responsibilities include the following: Coordinate, schedule and transport individuals to medical appointments. · Operates van or car as specified by agency procedures. Follows Motor Vehicle regulations to ensure the safety of people served. · Assists people served in entering and exiting agency vehicles, as prescribed by agency policy, and determined by the person supported needs. · Ensure wheelchairs are appropriately tied down following agency training. · Maintains vans for cleanliness and safety. Fuels vehicles, removes trash, wipes down spills, etc. Immediately report any malfunctions or safety hazards to the Transportation Coordinator. · Performs safety checks of the vehicle as prescribed by agency policy, documenting results in maintenance logs. · Records all trips according to vehicle logs. · Attend driver training and other in-services as required. · Maintains safe, orderly work area and work practices. The person served should not be left in the vehicle alone. · Serves as a liaison on everyone transported to medical appointments, communicating with practitioners about individuals current status, reason for appointment and ensuring that doctor documents and prescribed treatments, including medication orders/prescriptions, and schedules any needed follow-up appointment prior to leaving the office. · Coordinates/communicates with nursing personnel on any issues/concerns that need to be addressed on the appointment for anything beyond routing care. · Ensure individual is appropriately attired prior to departure and has all necessary consult sheets/medical records needed for appointment. · Help individuals in developing self-advocacy skills by facilitating discussions and role-playing scenarios (as directed by the Self-Advocacy Liaison). · Guide participants on how to communicate their needs and preferences effectively. · Performs a broad range of personal care and support activities for severely physically impaired individuals based on the individual’s capabilities. · Assists in recreational activities with individuals as part of prescribed group activities. Works with individuals to assist them in reaching established goals and objectives. Documents all activities and progress in appropriate logs. · Other duties may be assigned at agency/supervisors’ discretion. #IND1099 Qualifications Certificates and Licenses: A valid NY State Driver’s License, ability to drive and a driving record acceptable for agency insurance coverage are required. CDL License preferred. Additional Requirements: Remain current in all OPWDD and/or OMH, as well as agency-required trainings. Candidates must be cleared and maintain acceptable records under the NYS OPWDD and/or OMH mandated criminal background check process. Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process. CPR and First Aid training before completion of the evaluation. American Sign Language may be required, as indicated by site location. Must cooperate and participate fully with Jawonio and/or Justice Center and OPWDD during an investigation or inspection, including making verbal and written statements as requested by the person(s) doing the investigation.
