Position: UPMC Pain Management is actively recruiting for a Full-Time Medical Assistant to join our Falk Medical team. The selected candidate will have the opportunity to partner with Dr. Trent Emerick (Associate Chief, Chronic Pain Service) and Dr. Danielle Zheng in their efforts to provide multimodal pain care to the Central Oakland patient population. Responsibilities: Collaborates with care team members to reach population health management goals of improving patient access to care, reducing number of emergency visits,and decreasing readmissions.Telephonic outreach to patients discharged from the hospital or emergency room to schedule follow up appointments and review medications and treatment plans.Make referrals to members of the extended care team and/or notify provider of post discharge needs or discrepancies in plans of care.Participate in population health outreach to patients and assist with gap closureProvide health education to patients to help improve or maintain a healthy lifestyleActively promotes patient engagement through wellness guidance and self-management support and promotes patient engagement through wellness guidanceand self-management support and promotes patient health literacy through learned motivational interviewing and communication techniquesMust be responsible and demonstrate proficiency in all practice category specific tasksGuides workflow and/or assigns tasks to other clinical staff.Assist management staff with supervisory activities. Participates with creation and implementation of departmental goals, processes and procedures. Schedule appointments. Verify insurance coverageand patient demographics. Prepare and managing charts to ensure information completed and filed appropriately. Other front office responsibilities, as required.Prepare patient rooms, set up instruments and equipment according to department protocol. Clean exam/procedure rooms, instruments and equipment betweenpatient visits to maintain infection control. Clean according to scheduled maintenance program and keeping appropriate records. Manage inventory of supplies;restocking exam/procedure rooms. Check schedules and organizing patient flow. Accompany patients to exam/procedure room. Assist patients as needed with walking, transfers, dressing,collecting specimens, preparing for exam, etc. Collect patient information/history; taking vitals, performing screenings per provider guidelines. Relay instructionsto patients/families.Answering calls and providing pertinent information including patient education as directed.Perform phlebotomy, EKG's, and level one labs. Perform clinic procedures according to policy and procedure. Assist physicians with procedures. Documentpatient information into the medical record.Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health). Maintain strict confidentiality related to medicalrecords and other data. Comply with all UPMC Health System policies and procedures.Under the direction and supervision of a physician, administer injectable medication and vaccines when appropriate.*Performs in accordance with system-wide competencies/behaviors.*Performs other duties as assigned. Successfully complete the UPMC preceptor training and serve as a preceptor and mentor to newly hired team members.Must be responsible and demonstrate proficiency in all practice category specific tasks.
