Description POSITION SUMMARY This role works with a multi-disciplinary team to care for highly vulnerable patients in our medically monitored residential substance use disorder program(also known as 3.7R level of care). The case coordinator plays a critical role, providing care coordination and ongoing support for patients who have significant medical or psychiatric conditions. The case coordinator may also provide support across medical programs including MAT, Residential, Withdrawal Management, and Psychiatric services. Specific duties include daily check-ins, scheduling outside appointments, escorting patients to appointments, and care coordination. This role works closely with a team that includes addictions counselors, medical providers, nurses and peers to provide excellent care to patients in this program. The team’s philosophy is rooted in radical kindness, harm reduction, and trauma-informed care. ESSENTIAL JOB DUTIES Meet with each patient in the program to determine what medical support they need, including primary care, dental care, urgent care and specialty referrals Facilitate morning check-in group for 3.7R patients Coordinate with residential, medical and peer teams; attend team meetings as scheduled, including daily and weekly huddles Coordinate with external care teams Arrange for patient access to medical supplies and other equipment as deemed necessary by the medical team Escort patients to outside medical appointments in Fora Health Vehicles Assist medical providers in gathering patient’s medical and psychiatric concerns and referring to appropriate providers as needed. May be asked to support other case coordinators Complete accurate and timely documentation in patient charts Support patients in accessing resources based on insurance Request and send medical records as needed ADDITIONAL JOB RESPONSIBILITIES Attend staff meetings as scheduled. Additional trainings to be completed as assigned in a timely manner. Special projects and other duties as assigned. Participate in ongoing supervision and consultation. Engage in community outreach efforts SUPERVISORY FUNCTIONS This position does not have supervisory duties. Requirements QUALIFICATIONS EDUCATION AND EXPERIENCE REQUIRED Valid driver’s license for transporting clients in Fora Health Vehicles High School Diploma KNOWLEDGE, SKILLS AND ABILITIES REQUIRED Proficient in Microsoft Office Suite, specifically Outlook, Excel and Word Detail-oriented and organized Demonstrated effectiveness in presenting ideas, transmitting information, or convey concepts to individuals or groups of people of varying educational, cultural and experiential levels. Ability to maintain professional boundaries with patients, the community, and other staff members. Ability to work collaboratively with patients, other treatment program staff, team members, and to treat everyone with respect and dignity at all times. Written and organizational skills to complete all required documentation. Communicates information effectively in writing and verbally Strong organizational skills and attention to detail Displays initiative and problem-solving skills Basic knowledge of substance-use disorders OTHER REQUIREMENTS For employees with a history of substance use disorder, Fora Health’s policy requires certification of a minimum of two years of sobriety for this position. Ability to pass a pre-employment or for cause drug tests. Ability to pass DHS criminal background and DMV checks Position will require some amount of regular connection to work via cell phone to answer questions and respond to internal and external contacts. WORKING CONDITIONS Prolonged use of computer Position generally works in an indoor office environment with some travel between sites or to outside appointments. Position does not consistently require overtime. Adjustment of work schedule may be required. Possible exposure to communicable diseases which can encompass a variety of illnesses and infections, including COVID, tuberculosis, Hepatitis A, B, and C, and HIV. Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and families. Fast paced, high volume work environment.
