Overview This is a full-time position for a licensed social worker. The Medical Case Manager provides education, counseling, cognitive-behavioral interventions and problem-solving to a highly receptive outpatient population of sickle cell patients and their families in an environment that values social work services. Responsibilities Provide supportive counseling sensitive to understanding the impact of sickle cell, body changes, financial impact, social difficulties, financial resources, complicated coping. Be visible and accessible; i.e. attend multidisciplinary meetings, participate on appropriate Sickle Cell and community committees. Provide a forum for those with varied needs to obtain support; i.e. Create meaningful quality of life experience for those facing life threatening crises, i.e. Retreats, use complimentary therapy. Assist the Sickle Cell Clinic staff with staying energized. Participate in fundraising for new and existing projects. Qualifications Current, unencumbered licensure as a Licensed Baccalaureate Social Worker (LBSW). Two years of social work experience preferred. Experience with the MS Office Suite of products, including Excel. KNOWLEDGE, SKILLS AND ABILITIES: Eager to learn more about the social and medical support requirements of our practice population. Personal interest in health and wellness that will inform patient education for lifestyle choices. Critical thinking, prioritization, and organization skills that contribute to improving patient outcomes. Location : Location US-VA-Norfolk
