Home Reach Inc. is a progressive Supported Living Agency that provides residential habilitation services to adults with intellectual and developmental disabilities. Our mission focuses on assisting individuals in living as independently as possible!

We are seeking a dedicated and compassionate individual to join our team in providing exceptional care to individuals in a high-level care home in Idaho. The ideal candidate will be responsible for providing direct patient care to individuals with complex medical needs, ensuring that they receive optimal comfort, dignity, and assistance with daily living tasks.

  • Key Responsibilities:

    • Provide hands-on care for the client with various medical conditions, including but not limited to, physical disabilities, chronic illnesses, and cognitive impairments.- Assist with activities of daily living, such as bathing, dressing, feeding, logging food intake, and toileting.- Assist client with mobility, transfers, and positioning to promote comfort.- Aid in managing high-level care needs, such as administering medications- Closely observe client for any changes in physical, emotional, or mental health, and report changes through the proper channels.- Document daily activities, observations, and incidents- Ensure a safe and comfortable living environment by maintaining cleanliness.

      • Experience in care, home health, or a similar healthcare setting- Strong knowledge of patient care techniques- Ability to lift, transfer, and reposition client as needed- Ability to work well independently and as part of a team- Excellent communication skills- Compassionate, patient, and empathetic demeanor with the ability to handle the demands of high-level care.
