JOB SUMMARY

The Medical Escort is responsible for safely accompanying clients to and from medical appointments, ensuring timely arrival and return, providing assistance as needed during appointments, and maintaining a clear communication with clients, caregivers, and healthcare providers. This role supports individuals who may have physical, cognitive, or emotional needs and ensures their well-being and dignity throughout the medical visit process.

QUALIFICATIONS

  • Experience working with elderly individuals or people with disabilties.
  • Strong communication and interpersonal skills.
  • Ability to maintain professionalism and discretion.

PHYSICAL REQUIREMENTS

  • Ability to perform tasks such as pushing wheelchairs, assisting with transfers, and being on your feet for extended periods.

Responsibilities
  • Accompany clients to scheduled medical appointments, including transportation assistance when applicable.
  • Provide physical assistance (e.g., with walking, wheelchairs, transfers) as needed.
  • Ensure clients are signed in, understand appoitnment logistics. and are supported throughout their visit.
  • Communicate relevant information between healthcare providers and client caregivers or case managers.
  • Monitor and report any health or behavioral concerns observed during escort servics.
  • Maintain accurate records and documentation of escort services provided.
  • Escort must make four (4) copies of paperwork at the end of each appointment. These copies must be placed into the Doctor, Nurse Practitioner, DON, and patient coordinator mailboxes. The original copy of this paperwork must be given to the nurses station.
  • Follow all HIPAA and client confidentiality regulations.
  • Ensure safety, dignity, and respectful treatment of all clients at all times.
