JOB SUMMARY:

Join our dedicated team at Newquay Health Centre, where we are committed to providing excellent patient care in a supportive and friendly environment. We are currently seeking a proactive and compassionate Care Navigator to join our busy GP practice team.
As the first point of contact for many of our patients, you’ll play a key role in ensuring they receive the right care, from the right person, at the right time.
The ideal candidate would have proven experience within a busy administrative environment, and previous NHS employment would be an advantage.

Responsibilities

MAIN DUTIES OF THE JOB:

As a Care Navigator, you will support the practice team in delivering high-quality, patient-centred care. You will aid the smooth and efficient flow of patients by assessing and directing patients queries appropriately, assisting the clinical team by managing appointments and supporting our triage team to help patients access the most appropriate services whether that’s with a GP, nurse, pharmacist, or one of our community and specialist partners.

KEY RESPONSIBILITIES:

Greet and assist patients in a courteous and professional manner.
Manage appointment bookings and cancellations efficiently.
Handle online, telephone and face-to-face enquiries from patients and other health care providers.
Process incoming documents (e.g. hospital letters, test results.
Maintain patient records and ensure confidentiality at all times.
Assist with administrative tasks as required by the practice.
