As our Medical Records Coordinator, you play a vital role in supporting home health and hospice care by ensuring every patient record is complete, accurate, and accessible. You’ll bring order to complexity, working closely with clinical and administrative teams to keep documentation flowing smoothly and compliantly. Based on the job description, your purpose is clear: maintain the integrity, confidentiality, and accuracy of patient records while supporting a compassionate, high-performing care team. Key Responsibilities Maintain accurate electronic and paper medical records, ensuring all patient-related documentation is complete and properly filed. Send, track, and verify signatures for orders, plans of care, and face-to-face documentation. Assist with internal audits, accreditation surveys, compliance reviews, and records requests in alignment with HIPAA requirements. Collaborate with intake, operations, and CareM Support teams to ensure timely creation and maintenance of patient charts. Audit admission paperwork for Home Health and Hospice to ensure compliance with regulatory standards. Participate in team meetings, including quality assurance meetings, supporting overall program compliance. Requirements What We’re Looking For Prior home health and hospice medical records experience strongly preferred. High attention to detail and consistency in managing documentation. Strong organizational skills with the ability to coordinate across multiple teams. Professional, courteous communication — by phone, email, and in person. Ability to work independently with initiative, follow-through, and reliability. Comfort working with technology, including EMR systems and standard office tools. Ability to maintain confidentiality and adhere to HIPAA and company privacy standards.
