ESSENTIAL SKILLS & EXPERIENCE:

· Excellent attention to detail and accuracy.
· Good IT skills with experience of using clinical systems.
· Understanding of medical terminology.
· Ability to work independently and as part of a team.
· Commitment to patient confidentiality and data protection.

ROLE SUMMARY:

The postholder will be responsible for accurately summarising and coding incoming patient medical records into our clinical system. This is a key role in ensuring that patient records are complete, accurate and accessible, supporting safe clinical decision-making.

KEY RESPONSIBILITIES:

· Read and interpret patient medical records (paper and electronic).
· Summarise relevant medical history into the clinical system in line with practice protocols.
· Code diagnoses, significant past medical history, operations, and key medications using appropriate clinical coding.
· Ensure data quality, accuracy and confidentiality at all times.
· Liaise with clinicians and administrative staff to clarify information where needed.
· Contribute to maintaining up-to-date and accurate patient records.
