JOB DESCRIPTION

• Conduct comprehensive psychosocial assessments and develop intervention plans to address residents’ emotional, financial, and social needs in alignment with the care plan.
• Facilitate advance care planning (ACP) and financial assessments to help residents and families understand medical decisions and access appropriate financial support.
• Perform crisis and risk assessments (e.g., self-harm, violence, family conflicts), and intervene to maintain a safe, supportive care environment.
• Plan and coordinate discharge processes, linking residents and families with community services and ensuring a smooth transition back to home or community settings.
• Provide individual and family counselling to build resilience, strengthen coping mechanisms, and enhance overall psychosocial well-being.
• Participate actively in multidisciplinary team discussions to integrate social perspectives into holistic resident care.
• Maintain accurate case documentation, reports, and data entry in client management systems, ensuring confidentiality and compliance with regulatory requirements.
• Support departmental projects and community engagement initiatives to promote psychosocial care and resident empowerment.
