We are looking for an experienced, talented, and compassionate Hospice Social Worker for a full-time 40-Hour position in Hingham and Weymouth areas, MA. What will you be doing: The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through Croí Health. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges. To provide medical services to Hospice patients/and or family members reflective of the philosophy, mission, and standards established by the Agency. Responsibilities
The Hospice Social Worker will join an interdisciplinary team to provide counseling and targeted interventions for homebound patients and their caregivers receiving Palliative or Hospice services.
