Job Details Level: Experienced Job Location: Health at Home- Boynton Beach Branch - Boynton Beach, FL 33426 Position Type: Per Diem Job Category: Health Care Job Description Summary: The Medical Social Worker is responsible for the implementation of standards of care for medical social work services. Essential Job Functions/Responsibilities: A. Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the Clinical Manager or Case Manager. B. Carries out social evaluations and plans intervention based on evaluation findings. C. Maintains clinical records on all patients referred to social work. D. Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs. E. Provides information to patients or families/caregivers and community agencies. F. Serves as liaison between patients or families/caregivers and community agencies. G. Maintains collaborative relationships with Organization personnel to support patient care. H. Maintains and develops contracts with public and private agencies as resources for patient and organization personnel. I. Participates in the development of the total plan of care and case conferences as required. J. Assists physician and other team members in understanding significant social and emotional factors related to health problems. K. Participates in discharge planning. Qualifications Position Qualifications: 1. Graduate of a Master’s Degree program in social work accredited by the Council on Social Work Education required in the state of Florida. 2. Graduate of a Master’s Degree program in social work accredited by the Council on Social Work Education or graduate of a Bachelor’s Degree program if there is a Masters-prepared social worker already on staff. 3. Current license in the agency state of operation as a Social Worker required excluding the state of Florida. 4. Minimum of one year's experience in health care required. 5. Experience in a home health care preferred. 6. Demonstrates good verbal and written communication, and organization skills. 7. Possesses and maintains current CPR certification. Continuing Education Requirements: Organization personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, organization personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend mandatory educational programs. Environmental and Working Conditions Environmental Conditions: May be exposed to extremes of heat and cold in all weather conditions. Working Conditions: May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances. May be exposed or occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. OSHA exposure category: · Category I — Position includes tasks that involve exposure to blood, body fluids, tissues. Required Personal Protective Equipment: as required by working conditions.
