1 to 2 years hospice experience preferred.
Licenses required: SSW or LCSW
Location: Utah County

Medical Social Worker Responsibilities:

  • Provide supportive counsel aligning to the patient’s and family needs
  • Assisting patients and their families to understand, accept and follow medical recommendations
  • Help utilize community resources available for the patient and their family
  • Maintains collaborative relationships with organization personnel to support patient care
  • Assists physician and other team members with understanding significant social and emotional factors related to the patient’s health
  • Help integrate clinical guidelines, protocols and other metrics in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of care
  • Maintain clinical records on all patients referred to social work
  • Other duties as delegated by the Clinical Director/Supervisor

Job Type: Part-time
Pay: $20.00 - $25.00 per hour
