Description

This position exists to work independently and as part of a team responsible for reconciling patients' medication regimens during the admission process. Medication reconciliation is defined as obtaining a complete and accurate medication history with the involvement of the patient and/or family; comparison of a patient's home medications to the medications ordered on admission and identification of discrepancies, followed by a clarification of any noted discrepancies. Qualifications Education: High school diploma or equivalent. College courses desirable, but not required. Experience: Prefer two years in clinical setting. Pharmacy experience preferred. Licenses, Certifications and/or Registrations: Pharmacy Technician registration through Alabama Board of Pharmacy. National Technician Certification recognized by ALBOP
